APPLICATION FOR A FOREIGN WORKER’S MEMBERSHIP IN THE FRENCH HEALTH INSURANCE SYSTEM
TERMS OF USE

PURPOSE

The service offered here is designed to facilitate and reduce turnaround time for first-time registration of foreign-born individuals with the French health insurance system, through a 24/7, secure, user-friendly online service that is widely available and optimizes case processing and follow-up.

EMPLOYER’S OBLIGATIONS

Within this framework, the employer shall inform any foreign employees about what the French health insurance system does and how it works as soon as they are hired.

The employer shall give foreign employees a brochure about the process of registering with the French health insurance system and inform them that their contact information will be supplied by the employer to the local health insurance fund (“CPAM”) in order to facilitate the registration, membership, and entitlement application processes.

The information shall be supplied in a format that has been agreed upon between the employer and the local health insurance fund (“CPAM”) and shall contain, at a minimum, the following information:

- The employee’s last, first, and middle name(s)
- The employee’s date and place of birth
- The employer’s address
- The employer’s telephone number
- The employer’s email address

The employer agrees to supply the following documentary evidence:

- The membership application form
- An ID:
- For European Union nationals: a copy of their identity card or passport,
- For all others: copy of the passport page with identifying information + residency permit (“titre de séjour”) or long stay visa as proof of legal residency.

- A civil status document:
- A long-copy or short-copy birth certificate showing parents’ information, a multilingual birth certificate, or any other document issued by a consulate (original certificate of birth, individual civil status record, etc.) with a legible stamp as authentication.

Records drawn up in any of the following languages are accepted: Albanian, Croatian, Czech, Danish, Dutch, English, Finnish, German, Hungarian, Italian, Latvian, Lithuanian, Norwegian, Polish, Portuguese, Romanian, Slovak, Spanish, Swedish, and Turkish.

If the record is not drawn up in one of the languages listed above, it must be translated either by a translator or interpreter who is listed as approved by the French courts (“Tribunal de grande instance,” “Cour d’appel,” France’s “Cour de cassation”), or by the French embassy or consulate in the country where the document was issued.

No other translation will be accepted.

The local health insurance fund (CPAM)’s obligations

The local health insurance fund (CPAM) agrees to authorize companies which have accepted the terms of use to access the online pre-registration procedure for foreign workers.
The CPAM agrees to process applications for registration/membership for foreign workers in a timely manner once the completed form and all required documents have been received. In order to keep turnaround times down, the CPAM may communicate with the employer by email if any difficulties arise and to request any additional required information.

Once the registration/membership process is complete, the CPAM shall offer the employee an individualized service:

- declaring a primary care physician (“médecin traitant”);
- promoting the ameli - ameli santé – ameli direct account.

SHARING AND STORAGE OF DATA

The parties to this agreement agree to comply with confidentiality laws pertaining to the storage and treatment of any identifying information in their possession.
The parties shall comply with the provisions of French data protection law No. 78-17 of January 6, 1978 as amended. They hereby agree to implement the information system security policy and all related measures.

ASSESSMENT AND COORDINATION

The following quantitative and qualitative indicators shall be monitored:

- number of signed membership applications,
- number of foreign employees registered and added as members through this web service,
- registration/ membership turnaround times upon receipt of a complete application,
- overall rate and level of satisfaction of employers registered for this program.
